What do you have to do to get to Heaven in Catholicism?

I'm studying Catholicism and I just don't understand what exactly you have to do to be "saved" or "go to Heaven" in their faith. Do you have to be Baptized? What if you weren't as baby? What do you have to do if you like an adult to join the Catholic faith??
